Terms and conditions
I hereby for myself, my heirs, executors and administrators waive and release Nancy Freedman-Smith and Gooddogz Training and any officers, employees, agents, volunteers and supervisors working with/for Nancy Freedman-Smith and Gooddogz Training and/or the organization at whose facility I and my dog are training from any and all liability of any nature for injury or damage which I or my dog, or any member of my family or anyone accompanying me may suffer, including specifically, but without limitation, any injury or damage resulting from the action of any dog, and I expressly assume the risk of such damage or injury while attending any training session or any other function or while on the training grounds of the surrounding area thereto. In consideration of and as inducement to the acceptance of my application for training with Nancy Freedman-Smith and Gooddogz Training I hereby agree to indemnify and hold harmless Nancy Freedman-Smith and Gooddogz Training et al from any and all claims or claims by any member of any family or any other person accompanying me to any training session or function of Nancy Freedman-Smith and Gooddogz Training et al or while on the grounds of the training facility or the surrounding area thereto as a result of any action by any dog, including mine.

Class Description: Obedience/Problem Solving classes taught one time per week for 6 weeks. Lessons are approximately 1 hour in length. First session 1 hour with out the dogs. Gooddogz uses positive-reinforcement training methods!
I (print name) ________________________________________ wish to enroll my dog in the Group class training program under the direction of Gooddogz Training. I understand the program will cover basic on leash obedience commands as well as help me to solve or curtail my dog's behavior problems. I understand and agree that my success or failure in this program is based primarily on my willingness and ability to work with my dog at least 20-30 minutes every day.
REFUND POLICY
Should you or your puppy/dog develop a medical problem and be unable to complete the course, you will be refunded for those classes you have not completed or you may take the option of continuing in the next session. Gooddogz Training requires a letter from the dogs Vet or your doctor within 10 business days. Failure to provide letter within ten business days of informing Gooddogz Training will negate refund.
